Thoracic Spine Pain / Mobility Restriction

What is Thoracic Spine Pain?

The thoracic spine (mid-back) often develops stiffness from poor posture or repetitive stress. Limited thoracic mobility contributes to pain and compensations in the neck, shoulders, and lower back.

Common Symptoms

  • Mid-back stiffness or aching

  • Pain with twisting, lifting, or deep breathing

  • Poor posture or rounded shoulders

  • Secondary neck or shoulder discomfort

Physical Therapist Diagnosis

A PT assesses posture, spinal mobility, and rib movement. Movement testing during rotation and extension identifies restrictions.

Why it Happens

Prolonged sitting, poor posture, and limited activity commonly cause thoracic stiffness. Athletes in throwing or overhead sports may also experience overuse-related pain.

Why it Doesn’t Always Heal on Its Own

Stiffness tends to persist without intervention, leading to compensations that stress other regions.

Ideal Physical Therapy Treatment

Rehab restores mobility, strength, and improves posture.

Key strategies include:

  • Manual therapy and mobilizations to restore motion

  • Strengthening of postural muscles

  • Mobility drills (rotation, extension exercises)

  • Postural retraining for long-term improvement

Expected Outcomes

Most athletes and active adults see quick improvements in pain and mobility with therapy.
